Mar\u00eda Colomer, Head of Training at CODNIB and a dietitian-nutritionist, reflects on how consultations have changed, the role supplements now play and why, amid the noise of the <\/span><i><span style=\"font-weight: 400;\">fitness<\/span><\/i><span style=\"font-weight: 400;\"> market, it is still essential to return to one basic idea: nutrition first.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>In recent years, have you noticed a change in the type of consultations related to nutrition and <\/b><b><i>fitness<\/i><\/b><b>? Is there more focus on performance, aesthetics&#8230;?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 My perspective on physical activity and nutrition is somewhat biased, because I work with people who come precisely because they already do physical activity or because they have been advised to do it. The key message for me is that any dietary recommendation a dietitian-nutritionist can make is intended for a human being who is physically active; for a sedentary person, that is not health. It always has to go hand in hand. You need to do an hour of physical activity every day, and that does not necessarily mean going to the gym: it can be walking, climbing stairs&#8230; In consultation, what we see is that more and more people clearly understand that nutrition and exercise have to go together. Many come because they are not getting results. They join a gym but do not see changes in their body composition, they look the same. And that is where a fairly common misunderstanding appears: \u201cif I do sport, I\u2019m going to eat.\u201d But in reality, you have to eat in order to do sport, not do sport in order to eat. As for <\/span><i><span style=\"font-weight: 400;\">fitness<\/span><\/i><span style=\"font-weight: 400;\"> understood as joining a gym, there certainly is quite a <\/span><i><span style=\"font-weight: 400;\">boom<\/span><\/i><span style=\"font-weight: 400;\">, especially among teenagers, because many centres offer cheap memberships and they go a lot. But I\u2019m not sure to what extent it is more of a social centre than a real physical activity practice. I have my suspicions.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>What kind of user profile are you seeing? Has it changed compared with a few years ago?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 In my case, I work in a centre where there are also physiotherapists and a physical trainer, so the profile is very varied, of all ages, because we all eat. I have noticed one important change: very often the whole family ends up coming, or the partner\u2026 People understand that this also means changing your environment and surrounding yourself with a healthier context. They look for a social group that does sport: gym, running, cycling&#8230; or even at home. They bring their husband or wife&#8230; I have definitely noticed that change. Not so much the idea of \u201cI\u2019m going to do this just for myself\u201d or \u201cI\u2019m going to follow this diet I read about,\u201d but rather a more integrated view within a healthy environment.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>The consumption of supplements such as protein or creatine has become very normalised. Would you say they are now used out of real need or more as part of a trend?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 People now come in already telling you what they take. It is a bit like when an older person says: \u201cI take the blue pill or the orange one.\u201d Young people talk to you about the colours of what they bought as if we were supposed to know what it is, without even knowing whether it contains carbohydrates, only protein, or what its composition is. That reflects uninformed purchasing of products which, if not used properly, are not going to provide any benefit. When it comes to supplement recommendations, what needs to be very clear is that, if they are used, they should be free from sweeteners and colourings. If I am going to choose one with a nutritionist, who is the person guiding you in that sense, it should be neutral. Because, although we know that the authorised amounts of additives are safe, in the case of prolonged consumption we still have doubts about their effects on health, especially gut health. People buy flavoured products, but that is something to avoid. If you need it \u2014 and that is assessed by a nutritionist \u2014 it should be neutral, whether we are talking about protein or creatine, which are the most widely consumed. In fact, creatine has doubled in price because demand is so high. But taking more protein does not mean there will be more muscle. There has to be support, other nutrients, and protein can also be obtained through food. It is true that when we talk about specific compounds, such as creatine, you would have to consume a lot of food to reach certain doses. But with protein we have many foods that provide it: eggs, defatted nut flours, legumes, meat, fish&#8230; We can perfectly well achieve that post-workout snack using only real food.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>In this context, do you detect misinformation or use of these products without professional supervision?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 Yes, quite a lot. I would distinguish between a supplement and a complement. A supplement would be something you need when food is not enough, for example in an oncology patient who has no appetite. In healthy people, we are often talking more about a complement. Nutrition first, <\/span><i><span style=\"font-weight: 400;\">food first<\/span><\/i><span style=\"font-weight: 400;\">, and then see whether a complement is needed and which one. It might be protein, iron or something else, but that requires a more holistic assessment. Protein, for example, is very easy to use and can be helpful in specific cases: a teenager who will not be able to have an afternoon snack, someone who needs a meal after a competition and is looking for convenience&#8230; In those cases it can make sense. But for most people, if they did not want to use a commercial product, they could manage with food. What these products often provide is convenience: instead of cooking, you already have it sorted.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>How is this rise of <\/b><b><i>fitness<\/i><\/b><b> affecting the nutrition sector itself? Has demand for professionals increased? Is there malpractice or a trivialisation of advice?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 I am a dietitian-nutritionist, and now we often see professionals such as physical trainers or gym instructors who sometimes do not have a university qualification that gives them an overall view of physiology and human health, and yet still make recommendations without really knowing the person. I understand that for the general public that can seem quite logical: this person is here, they work in <\/span><i><span style=\"font-weight: 400;\">fitness<\/span><\/i><span style=\"font-weight: 400;\">, so they must know about this. But that is not necessarily the case. If we want nutrition advice, we should make sure it comes from a dietitian-nutritionist. There are different profiles: some focus exclusively on nutrition; others base the plan on nutrition but with complements. The important thing is that the public should make sure the person giving the advice actually has the knowledge. \u201cIt worked for me\u201d is not enough. In fact, in food advertising many comparisons that generate false expectations are prohibited.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>Are more people studying nutrition now?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 Yes. I graduated 25 years ago and back then there were only three universities. Now I think there are about twenty in Spain offering Human Nutrition and Dietetics. There is definitely growing interest in nutrition. In the public healthcare system there are hardly any dietitian-nutritionists, but even so there is more training on offer; here, for example, there is a private faculty affiliated with the UIB.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>From your experience, do you think we are at a point where nutrition is used more to build a body image than to take care of health?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 I would say that, in a very high percentage of cases, there is a motivation to lose body fat. But it is true that lately this comes with a healthier idea: not doing it with a beginning and an end, but as a change of habits. If people come to us, instead of doing it through an app, a book or someone else\u2019s diet plan, they are already people who understand that this is not just about a diet, but about habit changes to improve health. They want to feel healthy, have energy&#8230; A good diet is one that gives vitality, prevents disease and provides energy. A diet that restricts foods, that you can keep up for three months and then sends you back to square one, is not a good diet.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2014 <\/span><b>Creatine has become one of the most visible products of this <\/b><b><i>boom<\/i><\/b><b>. What should people be clear about when it comes to it?<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">\u2014 Creatine is a compound that we mainly find in meat, fish and eggs. But to reach the amount provided by a complement, you would have to eat a lot of food. In sport it is very common, because at the moment we have quite a lot of confidence in its use, so it is often used with people who exercise. Like any complement that becomes fashionable, the industry looks for new sales niches. If it used to be very focused on athletes, now they are trying to extend it to menopausal women, older people, and so on. It is a safe product, but that does not mean it is necessary for everyone. And above all, the first thing to work on properly is nutrition, because that is what is really going to bring about change. No matter how many complements you add, you are not going to achieve the expected result if the foundation is not right. In general, when you walk into a consultation, creatine is not recommended straight away. To parents, for example, we recommend that if their children want to use it, they should go to a professional. The fact that they want to look after themselves is already a very good starting point. What is missing is proper guidance. With one or two consultations, in healthy people, you can very simply draw up a meal plan and assess whether they need a complement or not, which one would be appropriate and how to use it reasonably. Before taking anything, the best thing to do is consult a nutrition professional who can assess what you actually need. Very often you order blood work and then see why there is a lack of energy or which nutrient is missing.
